Federal law requires Allerim to give you this Notice no later than first service delivery.

Allerim is committed to protecting the confidentiality of your protected health information (PHI) and to supporting privacy practices aligned with applicable healthcare standards.

Our Duty to Protect Your PHI

We are required by law to maintain the confidentiality of your protected health information and to provide you with this Notice of Privacy Practices.

We must follow the privacy practices described in this Notice, although we reserve the right to change our privacy practices and the terms of this Notice at any time.

How We May Use and Disclose Your PHI

We may use and disclose your PHI for the following purposes:

  • Treatment: to provide, coordinate, or manage your health care and related services.
  • Payment: to obtain payment for the health care services we provide to you.
  • Health Care Operations: to carry out quality review, training, and operational oversight.

Your Rights Regarding Your PHI

You have the following rights regarding your PHI:

  • Request restrictions on certain uses or disclosures.
  • Request confidential communications in a particular way or location.
  • Inspect and copy your PHI.
  • Request amendments to your PHI.
  • Request an accounting of disclosures.

Copies, Questions, and Changes to This Notice

You may request a paper or electronic copy of this Notice at any time, even if you agreed to receive it electronically.

We may update this Notice from time to time. The current version will be available in the portal and on request.

If intake asks you to acknowledge this Notice, that acknowledgment confirms receipt or the opportunity to review this Notice. It does not waive any of your rights.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services.
